• Request a Leave of Absence during term time (to be completed at least 4 weeks in advance)

• GDPR: Consent for photographs

• Use of emergency salbutamol inhaler
• Consent to administer emergency epipen

Years 3, 4, 5 and 6 specific

• Consent for child to walk to school unaccompanied: Year 5
• Consent for child to walk to school unaccompanied: Year 6
• Consent for child to walk to school unaccompanied from extra curricular activity: Year 6 (Autumn 1 and Summer) Year 5 (Summer only)
• Consent to watch PG films
